VitaCare Home Health Services Telehealth program main goal is to keep people with chronic illnesses out of the hospital. Equipment placed at you or your loved ones home transmit electronic data to skilled clinician who constantly throughout the day monitor patients vital signs seven days per week.
Patients with the most benefit with this program include patients with chronic illnesses such as: Congestive Heart Failure, Chronic Obstructive Pulmonary Disease, or Diabetes among other conditions.
The programs mission is to reinforce behavior and lifestyle changes and including provide patients and their families with peace of mind knowing a clinician is constantly monitoring their health and respond immediately if issues arises/ or needed.
Patient in our telehealth program need constant Blood Pressure Monitoring, heart rate, oxygen saturation and weight. If our clinicians see something outside the normal , they contact the patient, notify your primary care physician and a nurse will visit the patient if needed.
